Suboxone, buprenorphine and Vivitrol providers in Maryland

Suboxone doctors in Maryland may practice in primary care, addiction medicine or psychiatry settings outside OTPs. Search FindTreatment.gov by ZIP code for buprenorphine providers in Maryland, or ask our navigation line about medication options. If you are considering Vivitrol in Maryland, ask a prescriber about eligibility and the opioid-free period required before starting.

Learn what an opioid treatment program does.

Listings reflect the CMS Medicare Opioid Treatment Program Providers file dated September 8, 2026. Confirm hours, current intake and accepted insurance with each location.

MAT Treatment Options in Maryland by Medication

Methadone clinics in Maryland

The CMS file lists 78 Medicare-enrolled methadone OTP locations in Maryland, including 30 in Baltimore City. Methadone for OUD is dispensed through certified OTPs; new patients often attend frequently at first, with take-home medication determined individually. People in counties without a listed OTP can compare nearby cities in the limited-access section. Learn about methadone treatment and how OTPs work.

Buprenorphine and Suboxone doctors in Maryland

Maryland residents may access buprenorphine treatment in qualifying office-based practices and some telehealth settings. Clinicians with appropriate prescribing authority do not need the former federal X-waiver, which ended in 2023. Maryland Medicaid includes buprenorphine within federally required OUD medication coverage, although individual prescriptions and network rules may vary. Read the buprenorphine and Suboxone guide and telehealth MAT overview.

Long-acting injectable buprenorphine: Sublocade and Brixadi

Sublocade and Brixadi are extended-release buprenorphine injections administered by qualified healthcare professionals. In Maryland, patients can ask an addiction-medicine provider whether an injection fits their treatment plan and which pharmacy or clinical benefit applies. These treatments differ in their dosing schedules and requirements, so a consultation matters. Learn more about Sublocade and how long treatment may last.

Naltrexone and Vivitrol in Maryland

Vivitrol is an extended-release naltrexone injection and does not contain an opioid. Before starting it, a person needs an appropriate opioid-free period to avoid sudden withdrawal. Maryland Medicaid includes naltrexone under the OUD medication coverage requirement, but the exact product and authorization steps depend on benefits. Compare naltrexone and Vivitrol with other OUD medications.

Methadone (OTP) Rules in Maryland

Maryland OTPs are licensed by the Maryland Department of Health and follow federal SAMHSA and DEA requirements under 42 CFR Part 8. Someone starting methadone generally completes an assessment, medication review and treatment plan with the program; ask the chosen Maryland clinic how soon an intake appointment is available.

Take-home methadone, transfers and guest dosing

Federal OTP rules updated in 2024 give clinicians more flexibility when deciding take-home doses; availability still depends on individual safety and applicable Maryland requirements. If moving between Baltimore and another Maryland city, ask about transfer records and guest dosing before travel. An OTP can explain its approval process and what documentation is required.

What to bring to an intake visit

  • Photo identification and any Maryland Medicaid, Medicare or private insurance card you have.
  • Current medication list, relevant medical information and previous treatment details.
  • Questions about transportation, dosing schedule, take-home eligibility and potential costs.
Good to know: Medication treatment does not need to wait for a person to complete counseling. See counseling and MAT and MAT versus MOUD.

Paying for MAT in Maryland

Maryland Medicaid (HealthChoice)

Maryland Medicaid, including HealthChoice, operates through the Maryland Department of Health. Maryland implemented Medicaid expansion on January 1, 2014, according to KFF. Federal Medicaid law requires coverage of methadone, buprenorphine and naltrexone for opioid use disorder; ask your plan about a particular clinic, formulation and any authorization.

ServiceCoverageNotes
Methadone for OUDYesUse a participating OTP; confirm plan and site
Buprenorphine for OUDYesSpecific brand, prescriber and pharmacy rules can vary
Naltrexone for OUDYesConfirm injection or pharmacy benefits

See Maryland Medicaid for program information.

Medicare

Medicare Part B has an OTP benefit for eligible Medicare patients receiving covered OUD treatment at enrolled programs. Take-home prescription medications outside the OTP may instead involve Medicare Part D or another applicable benefit. Baltimore-area and Eastern Shore clinics should confirm enrollment and billing before a visit.

Private insurance

Private health plans may cover Maryland addiction treatment, but the provider network, cost-sharing and specific medication can differ. Ask both the insurer and clinic whether the prescribed medication and visits are covered; mental health parity rules do not guarantee a particular provider or prescription.

No insurance or limited ability to pay

The Maryland Behavioral Health Administration funds behavioral health treatment through local providers. Ask a local clinic about eligibility, available public funding and payment arrangements, or look for a nearby community health center. Review insurance coverage, verification questions and MAT cost information.

Telehealth MAT in Maryland

Telehealth Suboxone in Maryland may be available through appropriately licensed clinicians, depending on the visit, medication and current rules. Federal DEA/HHS controlled-medication telemedicine flexibilities run through December 31, 2026; Maryland requirements and prescriber policies may also apply. A virtual visit does not automatically mean a prescription or same-day start.

Methadone for OUD still involves an authorized OTP for dispensing, even if some assessment or follow-up components use telehealth. Ask a Maryland buprenorphine practice whether it offers initial visits virtually, how prescriptions are handled, and when an in-person appointment is needed. Read our telehealth MAT guide.

Policy checked: October 2026. Telemedicine regulations can change after this date; confirm current federal and Maryland requirements with the clinician.

Opioid Overdose in Maryland: Why Access Matters

Maryland recorded 1,772 drug overdose deaths in 2024, with an age-adjusted all-drug rate of 26.9 per 100,000, according to CDC NCHS data reported through USAFacts and KFF. The rate declined from 44.6 in 2020, but remained above the national 2024 rate of 23.1. Maryland’s 2024 opioid-involved death rate was 22.4, compared with 16.0 nationally.

MAT for Special Populations in Maryland

Pregnant and postpartum people

A pregnant or postpartum person in Maryland can ask an obstetric clinician and an addiction specialist about methadone or buprenorphine treatment. The right choice depends on individual medical needs, and stopping opioid medications without medical guidance may be unsafe. Ask a Maryland OTP or outpatient prescriber about coordinated prenatal and postpartum care.

Veterans

Veterans in Maryland can ask their VA care team about evaluation for opioid use disorder and referrals for medications. VA and community-care eligibility depend on the individual. If an appointment involves an outside Maryland OTP, check how coverage and transfer records will work.

Adolescents and young adults

Young people in Maryland should seek an age-appropriate assessment for opioid use disorder, including medication options when clinically appropriate. Families can ask about consent, confidentiality and youth-specialist referrals. Maryland’s Good Samaritan law also provides limited protections involving three underage-alcohol offenses when people seek emergency help.

People leaving jail or prison

Continuity of OUD medication can matter when someone returns to a Maryland community from incarceration. Before discharge, ask the facility’s care coordinator about records, the next prescription or OTP appointment, Medicaid enrollment and naloxone. A Baltimore or other county OTP can explain its intake documentation requirements.

Residents of Garrett, Kent, Caroline, Dorchester, Somerset and St. Mary’s counties can compare nearby OTP cities with office-based buprenorphine options. Telehealth may help with eligible prescribing visits, while free naloxone training is available through Maryland Overdose Response Programs. Transportation plans are especially important for frequent OTP visits.

How to Choose a MAT Clinic in Maryland

The most useful Maryland clinic is one that offers the medication you need, a workable schedule and payment arrangements you understand. Compare at least a few local programs when you can, especially if you travel from an unlisted county.

Questions for methadone OTPs

  • Is your Maryland OTP accepting new patients?
  • What identification and records do I need for intake?
  • How often are dosing visits required at first?
  • What is your policy on take-home doses or temporary guest dosing?
  • Does this location take Maryland Medicaid, Medicare or my insurance?

Questions for buprenorphine providers

  • Do you prescribe buprenorphine or provide long-acting injections?
  • Do you offer in-person care, telehealth or both?
  • What is the first-visit assessment and follow-up schedule?
  • Which plans, pharmacies and medication formulations are accepted?
  • How do you handle side effects, missed appointments and continuing care?

What happens at the first visit

  1. Share your health history, medication list, goals and current concerns.
  2. Discuss available medications and the safest way to begin treatment.
  3. Confirm the follow-up plan, transportation, payment and emergency contact instructions.
Red flags: Be careful of any program that guarantees a result, demands unexplained large cash payments, or insists that counseling alone must replace consideration of medication treatment. Consider helping a loved one prepare questions.
